TEST RESULTS

Ambucare Clinic provides its Occupational Health clients with the option of checking drug test results for their employees. These can either be emailed to you or they can be accessed via the web. Comprehensive reports are also available online.

Clients wanting to sign up for these services must fill out a form. These forms are in PDF format. Please fill it out and fax it to 765-668-2786 to the attention of George Earnest. If you have any questions regarding filling out the form, please call George at 765-664-0511.
